4/5/2023 EnchantaVaughn Approved (AM)Upon arrival, client was observed settling into his morning routine and waiting for QMHP-C to start the morning group. Client was in a calm mood, as evidenced by his relaxed demeanor and quiet tone. Client displayed good conduct in academic setting by reading and reviewing with his group. Client was engaged and attentive despite others who were being disruptive in class. Client separated himself from negativity to avoid conflict. Client contributed to class discussion expressing positive thoughts about the lesson. Client is seen demonstrating excellent involvement with his peers and working as a team. Client displayed full participation in team building exercise and no redirection was needed at this time. Client appeared to be enjoying his time in class, as evidenced by his positive body language and positive interactions with classmates in the Spring Break setting. Client reframed from aggressive and impulsive behaviors and praised for controlling outbursts. (PM)Client has been staying out of trouble and making good choices. Client was monitored and supported for complying to school rules and displaying excellent cooperation in lunch, encore, and select electives. Client has been doing a wonderful job with reframing from running, jumping, and hitting when playing in a respectful manner with his peers.. (PM) QHMP-C reported that client has been improving a lot and she’s very proud of him. Client is respectful and cooperative while completing his work and careful not to interrupt anyone else working quietly next to him. Client has made good choices on his own and shows that he is capable of being good and treating others fairly. Client seems to be enjoying himself in academic setting, as evidenced by the positive energy displayed around his classmates and QMHP-C.
